FACTS Grant & Aid Assessment

​Saint Theresa of Avila School utilizes the FACTS financial aid assessment service. If you wish to apply for financial aid, please complete the FACTS Grant and Aid Assessment application online. The fee is $35.

1. Complete the FACTS Grant & Aid Assessment application by visiting www.factsmgt.com.  

2. All required tax documents must be submitted for your application to reach the "verified" status. Your application must be verified before a financial aid award is made. The verification may take up to two weeks to process. The following tax documents must be submitted:

    • Copies of your most recent tax returns (IRS 1040, 1040A, or 1040EZ).  This includes supporting tax schedules C, E, and F.

    • Prior year W-2s for all parents/guardians.

    • The above-mentioned documents should be submitted by the following methods only. These submission methods are listed in the order in which they will be processed the quickest:

      • Upload documents directly to your application

      • Fax your documents, noting your application ID number, to FACTS at 866-315-9264

      • Send copies via US Mail.

      • Note: Emailed documents will not be accepted.

3.  FACTS Grant and Aid Assessment application must be completed by April 1.  

4.  Submit only one application per household.

5.  All families currently receiving financial aid must apply each year.

6.  All household income must be reported.
